Pre-participation Physicals will be held in the Sports and Medicine Complex at the Northport Medical Center at 9:45 a.m.  There is a nominal fee of $15 or $10 and a piece of usable equipment to be paid in cash by the athlete at the time of the physical.  This money will benefit the DCH SportsMedicine Fund, a part of The DCH Foundation.  Athletes who do not have payment the day of physicals will not be given a physical exam.  Athletes should bring cash only and exact change if possible. The medical history section of the physical form must be filled out and signed by the athlete AND his or her parent/guardian.  Please fill out forms in black or blue ink.
